car overreving help

hey guys i got a 04 celica gt. just hit 50k miles. and when i drive it when goin either from 1st to 2nd or 2nd to 3rd etc. the car takes forever to respond and speed up and it over revs way high from 2k all the way up to 6k without going anywhere for maybe 5 seconds or so. any input?:confused:
