Rough Coast

I noticed this lately, it happens particularly strong in lower gears.

If I keep my foot steady on the gas and not accelerate/decelerate, the car seems to jerk, as if I am removing my foot from the throttle and then pressing it back in.

What could it be? and how do I fix it?
